Free Printable Pet Health Record

I know that there are lots and lots of pet owners that have a great desire to take good care of their pets. I’m not only talking about making sure that they get the right amount of exercise and the perfect nutritional foods… One thing that is equally important is to keep a pet health record. Some records can be bought at the vet while others are given for free.

Pet Health Record

However since you don’t want to visit the vet more than you have too (because it’ll cost you every time), there is a need for online downloads of a free printable pet health record. I have previously written a post on that specific topic, but I thought that it was time to bring up the subject once more.

Pet Health Forms

If you would like to read my previous article you can do so here. In that article I suggest that you go download the health form that Microsoft provides as a part of the Office package. However I am working on releasing my very own PetInsurancePro pet health record. If will of course be available for free download.

i have a puppie who is 8 weeks old but i do not know if she was vaccinated. Can i just start fresh as her first vaccine?

    Vanessa,

You can always call the people you got her from to see if and when they vaccinated. A responsible breeder would have vaccinated and dewormed all puppies.

If you found her (or bought her at a local market like a farmer’s market or something), then chances are good that she probably hasn’t been vaccinated. In that case, I’d take her to the vet so that they can do a full exam and check for worms and everything too. They can then give her a set of shots and you can do all of the follow up shots. Ask your vet how to give the shot properly. If your vet won’t do this or you aren’t comfortable with what your vet is doing, find a new vet. Your relationship with your vet (even if you don’t go very often) is VERY important. You need to be comfortable with them. Would you take your child to a doctor that you don’t feel comfortable with? Same concept.

Good luck with your new puppy!! :)

you can also find a very nice template on the microsoft website:

    http://office.microsoft.com/en-us/templates/results.aspx?qu=pet&AxInstalled=1&c=0#ai:TC006128278|

it will open in Excel, and you can change it anyway you need to. like i change the clipart picture from a cat to a dog that looked more like my own.

    hope this help.

Hi, I just got a black lab from a man who was sent overseas and I can now not contact. What is considered “up to date vaccines?” I am hiring a trainer and this is a requirement. The previous owner got the dog from the SPCA neutered and all vaccines as a puppy, but I have no idea what if anything else he did. I do have a “general” idea where his vet was but wonder if I’ll have issues getting his records transferred to my vet. I do not even know the man’s last name. It would be on the microchip I would think, but I don’t know if the SPCA will even change that w/o some sort of permission from the previous owner. I had no idea the previous owner would up and leave this soon and not return my calls.
